Job Responsibilities
- Provide direct nursing care as prescribed by the physician to acutely ill or chronically ill patients in their homes.
- Record clinical and progress notes.
- Coordinate other patient care activities/services to provide the highest quality of patient care.
- Work under the direct supervision of the Registered Nurse and/or designated supervisor.
- Provide high quality clinical services within the scope of practice as defined by state laws.
- Document patient visits thoroughly and completely in the electronic medical record.
- Participate in the agency's care coordination process including timely reporting of patients’ needs and changes in condition.
- Ensure confidentiality of patient health information.
Required Skills
Core Skills
- Current LPN licensure
- Ability to document patient visits in electronic medical records
- Effective communication skills
Preferred Skills
- Previous home care experience
Requirements
Experience: Minimum one year of experience working as a LPN
Education:
License & Certification:
- Current LPN licensure in state of practice
- Current CPR certification
- Current driver's license and vehicle insurance
Schedule:
- FT/PT hours available
