Job Responsibilities

Collaborate, communicate & facilitate and document optimal provision of care through a computerized and/or digital platform supporting daily management of care.
Assess, plan, implement, coordinate, monitor & evaluate the patient's plan of care from admission to discharge.
Organize care through collaboration with the multidisciplinary care team and participate directly or indirectly with patients.
Utilize clinical knowledge and critical thinking skills to coordinate, implement & support the plan of care.
Ensure a seamless, effective, efficient transition of care across the continuum.
Perform chart audits as needed to enhance quality and safety.
Monitor quality metrics specific to the department & ensure complete & accurate documentation in the patient record.
Perform virtual Safety Rounds.
Demonstrate a high degree of professional competence and autonomy as a clinician, educator, consultant/collaborator, and navigator.

Required Skills

Core Skills

Baccalaureate Degree in the Science of Nursing (BSN)
Current licensure by the Maryland State Board of Nursing or Compact State as a Registered Nurse
Minimum five (5) years of RN experience in an acute care setting
Electronic Medical Record (EMR) experience
