Job Responsibilities

Responsible for collaboration, communication & facilitation and documentation of optimal provision of care through a computerized and/or digital platform supporting daily management of care.
Responsible for the assessment, planning, implementation, coordination, monitoring & evaluation of the patient's plan of care from admission to discharge.
Organize care through collaboration with multidisciplinary care team and participates directly or indirectly with patient.
Utilizes clinical knowledge and critical thinking skills to coordinate, implement & support the plan of care.
Ensures a seamless, effective, efficient transition of care across the continuum.
Performs chart audits as needed to enhance quality and safety.
Responsible for monitoring quality metrics specific to the department & ensuring complete & accurate documentation in the patient record.
Performs virtual Safety Rounds.
Demonstrates a high degree of professional competence and autonomy in performing various roles such as clinician, educator, consultant/collaborator, and navigator.

Required Skills

Core Skills

Baccalaureate Degree in the Science of Nursing (BSN) from an accredited school of nursing.
Current licensure by the Maryland State Board of Nursing or Compact State as a Registered Nurse.
Minimum five (5) years of RN experience in an acute care setting with adult condition management.
Electronic Medical Record (EMR) experience
