Position: Licensed Practical Nurse

Pay: Case Specific Enhanced Pay Rates available, Night shift and weekend shift differential pay offered

Job Responsibilities
- Provide direct nursing care as prescribed by the physician to acutely ill and/or chronically ill patients in their homes.
- Record clinical and progress notes.
- Coordinate patient care activities/services to provide the highest quality of patient care.
- Work under the supervision of the Registered Nurse and/or designated supervisor.
- Provide high quality clinical services within the scope of practice according to state laws.
- Document patient visits thoroughly in the electronic medical record and electronic visit verification program.
- Participate in the agency's care coordination process including timely reporting of patient needs and changes in condition.
- Attend patient case conferences and communicate effectively with patients, families, physicians, and other clinicians.
- Ensure confidentiality of patient health information per agency policy.
Required Skills
Core Skills
- Current LPN licensure in state of practice
- Ability to document patient visits thoroughly
- Effective communication skills
- Ability to coordinate with healthcare team
Preferred Skills
- Previous home care experience
- Current CPR certification
Requirements
Experience: At least one year of experience working as an LPN preferred
Education:
License & Certification:
- Current LPN licensure in LA
- Current driver’s license
- Vehicle insurance
- Access to a dependable vehicle or public transportation
- Current CPR certification
