Job Responsibilities
- Coordinate all aspects of the patient's care under the supervision of a Clinical Manager and attending physician.
- Identify patient/family needs and provide supportive care as per physician's orders and plan of care.
- Work with the interdisciplinary team to plan and coordinate care.
- Participate in ongoing comprehensive assessments, case conferences, and coordinate the plan of care.
- Facilitate discussions in interdisciplinary team meetings regarding issues from caseload.
- Supervise and direct others in planning and implementation of patient care.
- Promote harmonious working relationships among team members.
- Collaborate with other hospice staff, volunteers, attending physicians, and outside agencies to optimize patient care.
Required Skills
Core Skills
- Registered Nurse, currently licensed in Massachusetts in good standing.
- Ability to assess and respond to the needs of patients and families.
- Self-directed, flexible, and cooperative in fulfilling role obligations.
- Effective collaboration within an interdisciplinary team.
Preferred Skills
- Associate's degree in nursing.
Requirements
Experience:
Education: Associate's degree in nursing required.
License & Certification:
- Registered Nurse (RN) license in Massachusetts.
