Obtain a nursing history and identify status, changes, and physical, social and psychological needs of the patient, family, and significant others.
Contact and consult with providers, providing nursing and bio-psychosocial history for admission orders.
Develop treatment and discharge plans, and coordinate home care needs.
Assume responsibility for quality of patient care by continually assessing the patient and modifying nursing care as needed.
Medicate per provider orders and document patients' response to care.
Develop treatment plans that reflect the assessed needs, strengths, preferences, and goals of the individuals served.
Re-evaluate and revise goals and objectives as necessary.
Discuss areas that differ from the usual standard of care with Providers, Nursing leadership, and/or Clinical Director.
Increase patients' knowledge and acceptance through continuous teaching regarding procedures, medications, discharge instructions, etc.
Oversee quality and continuity of care for patients, accepting individual responsibility for compliance with unit protocols.
Maintain accountability for nursing practice within an ethical/legal framework.
Admit new patients and determine appropriateness for admission and program placement based on ASAM criteria.
Apply the nursing process effectively in the care of culturally diverse patients and families.
Facilitate weekly psychoeducational groups as assigned.

Experience: Minimum of two years current experience in acute care nursing.

Education: Associate degree in nursing required, Bachelor degree in nursing preferred.
