Job Responsibilities

Collaborate, communicate, and facilitate optimal provision of care through a computerized and/or digital platform supporting daily management of care.
Assess, plan, implement, coordinate, monitor, and evaluate the patient's plan of care from admission to discharge.
Organize care through multidisciplinary care team collaboration and participate directly or indirectly with patient.
Utilize clinical knowledge and critical thinking skills to coordinate, implement, and support the plan of care.
Ensure a seamless, effective, efficient transition of care across the continuum.
Perform chart audits as needed to enhance quality and safety.
Monitor quality metrics specific to the department and ensure complete and accurate documentation in the patient record.
Perform virtual Safety Rounds.
Demonstrate a high degree of professional competence and autonomy in performing various roles such as clinician, educator, consultant/collaborator, and navigator.

Required Skills

Core Skills

Baccalaureate Degree in the Science of Nursing (BSN)
Current RN licensure by the Maryland State Board of Nursing or Compact State
Minimum five years of experience in an acute care setting
Experience managing adult conditions
Electronic Medical Record (EMR) experience

Education: Baccalaureate Degree in the Science of Nursing (BSN) from an accredited school of nursing.
