Job Responsibilities

  • Assess and determine health status and level of care for all new admissions.
  • Ensure accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, and significant change.
  • Communicate level of care for new resident to all disciplines.
  • Coordinate interdisciplinary participation in completing the Minimum Data Set (MDS) for each new admission according to regulatory time frames.
  • Maintain an accurate schedule of all MDS assessments with proper reference dates throughout resident’s stay.
  • Perform data entry for accurate electronic submission of MDS assessments.
  • Verify electronic submissions of MDS, perform necessary corrections, and maintain appropriate records.
  • Schedule and conduct resident care conferences in compliance with state and federal regulations and ensure completion of all MDS reviews prior to conferences.
  • Assist disciplines in formulating and revising care plans, identifying problems, establishing goals, and ensuring appropriate nursing interventions.
  • Evaluate resident care plans for comprehensiveness and individuality.
  • Assess achievement or lack of achievement of desired outcomes and revise care plans appropriately.
  • Responsible for all level of care changes within the facility and notify all departments of any changes.
  • Generate appropriate forms for level of acuity changes and transmit forms to the appropriate agency as required.

Required Skills

  • Registered Nurse with current unencumbered state licensure
  • Long Term Care Experience
  • Ability to read, write, speak, and understand English
