Job Title: RN Clinical Manager

Position: Clinical Manager overseeing home health patient services

Job Type: Full-time

Pay: $77,200 - $106,200 per year

Shift: M-F / 8 AM - 5 PM (ET)

Job Responsibilities

  • Coordinate and oversee all direct care patient services provided by clinical personnel.
  • Focus on home health clinical quality assurance and clinical operations initiatives.
  • Develop, plan, implement, analyze, and organize clinical operations for the location managed.
  • Conduct and delegate assessments and reassessments of patients, updating care plans and interpreting patient needs.
  • Manage the assignment of caregivers and oversee delivery of care to all patients served by the location.
  • Receive case referrals and determine home health or hospice needs based on available patient information.
  • Ensure patients meet admission criteria and make decisions to admit patients to services.
  • Assign appropriate clinicians to cases as needed.
  • Instruct and guide clinicians to promote effective performance and quality home care services.
  • Assist clinicians in establishing therapeutic goals and developing patient Plans of Care (POC).
  • Monitor cases for compliance with documentation and regulatory agencies.
  • Ensure audits and billing are completed timely and in compliance with Medicare regulations.
  • Coordinate communication between team members, attending physicians, and caregivers.
  • Work with Branch Director and Finance Department to establish revenue and budget goals.
  • Participate in sales and marketing initiatives.
  • Supervise clinical employees assigned to the location.
  • Carry out supervisory responsibilities according to Company policies and procedures.
  • Handle employee corrective actions and discipline issues fairly and objectively.
  • Participate in the interviewing, hiring, training, and development of direct care clinicians.
  • Evaluate staff performance relative to job goals and recommend in-service education programs.
  • Assess staff education needs and conduct regular education as needed.
  • Analyze problems and evaluate alternative action courses using Performance Improvement principles.
  • Review Case Managers and clinical staff documentation for appropriateness and compliance.
  • Manage QA/PI activities and work with Utilization Review on data tracking.
  • Perform patient care assignments on an infrequent basis during emergencies.
  • Act as Branch Director in absence.
  • Interpret Company standards and policies to ensure regulatory compliance.
  • Participate in performance improvement activities and maintain clinical knowledge.
  • Maintain relationships with referral sources and participate in professional organizations.
  • Conduct care-related programs and perform other duties as assigned.

Required Skills

Core Skills

  • Registered Nurse with a current state license
  • Management and leadership experience in home care or hospice
  • OASIS experience
  • CPR certification
  • Valid driver's license and reliable transportation
  • Ability to analyze situations and identify problems

Preferred Skills

  • OASIS certification (HCS-O, COQS, and/or COS-C)
  • Homecare Homebase (HCHB) experience
  • CMS PDGM billing knowledge or experience

Requirements

Experience: Two years as a Registered Nurse with at least one-year of management experience in a home care, hospice or equivalent environment.

Education: Graduate of an accredited School of Nursing.

License & Certification:

  • Current state license as a Registered Nurse
  • CPR certification

Supplemental Pay
