Job Responsibilities

Work directly with the clinical and sales team to identify member external appointments and facilitate access to those providers in a seamless and timely manner.
Use facility EMRs to prescreen new enrollees to determine any specific continuity of care needs for external appointments/specialists and facilitate their access.
Perform care coordination activities, including member transitions of care communication to and from facility.
Assist with Prior auth requests process and members needing out of Nursing Facility care and services.
Access the drug formulary, provider directory and communicate with member patients/families and doctors as delegated by team Nurse Practitioners.
Participate in Longevity Health Plan NC standing clinical meetings on a daily/weekly/monthly basis.
Gather data for hospitalization review sessions.
Assist with onboarding new nursing homes and educate team members on LHP model.
Assist with the initiation of telehealth service in newly onboarded NHs, provide education, and follow up as needed.
Conduct nursing home orientation sessions for all shifts on LHP MOC and introduction to the model in new and existing homes.
Assist with LHP EMR data input as requested.
Perform other duties as assigned based on caseload and workload.

Required Skills

Core Skills

Licensed registered professional nurse in good standing
Knowledge of managed care coordination
Proficient user of LHP EMR
Excellent verbal and written communication skills
Strong computer skills, including EMR/clinical platform and MS Office products

Preferred Skills

Nursing home background or health plan case management experience
Minimum 2 years experience as a Registered Nurse
Knowledge of challenges and opportunities in serving the long-term nursing home population
Ability to travel

Requirements

Experience: Minimum 2 years experience as a Registered Nurse

Education: Associate degree in nursing required; bachelor's degree in nursing or other healthcare-related discipline preferred.

License & Certification:

Valid state driver's license
Proof of automobile insurance
