Job Responsibilities
- Performs comprehensive nursing history and physical on new referrals.
- Creates appropriate care plan and participates in IDT.
- Evaluates and triages participants with episodic illnesses.
- Refers participants to medical director or medical specialist as indicated.
- Actively participates in family conferences.
- Participates in interdisciplinary team meetings, committees and work groups as assigned.
Required Skills
Core Skills
- Bilingual English/Spanish
- Proven clinical leadership
- Ability to work in an interdisciplinary team environment
Preferred Skills
- Knowledge of Creole
- ACLS certification
Requirements
Experience: At least 1 year of RN or 3 years of LPN required. Minimum one-year experience working with a frail/elderly population.
Education: Graduate from an accredited school of nursing.
License & Certification:
- Current RN license by the Florida State Board of Nursing
- Valid Florida Driver’s License & Proof of Insurance
- BLS certification required
