Plan, organize, develop, and direct community clinical and medication programs.
Inform and collaborate with the Executive Director on clinical concerns and compliance issues.
Serve as a clinical resource for community staff.
Provide nursing care to residents, including health assessments and completion of resident Care Plans.
Discuss changes in resident condition with physicians and introduce interventions to reduce falls.
Administer medications and assist with resident care as needed.
Accountable for timely assessments and accurate care plans in the electronic health record.
Communicate with the Business Office Manager regarding additional care given for accurate resident billing.
Maintain knowledge of state regulations, policies, and procedures for compliance.

Licensed Registered Nurse (RN) preferred, Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (Texas)
Current Nursing License for the state of practice
Geriatric assessment skills
Knowledge in geriatric and dementia care
Ability to train staff and assess systems

Experience: Minimum 1-2 years of management experience in community, acute, skilled, or long-term care settings

Education: Bachelor’s degree in Nursing, Social Work, Psychology, Gerontology preferred
