Job Title: RN Clinical Manager, Home Health
Position: Clinical Manager overseeing patient services and operations in home health
Job Type: Full-time
Pay: $77,200 - $106,200 per year, annual bonus eligible
Shift: Normal Hours of Operation: M-F / 8a-5p (ET), On-Call Expectation: Yes, rotating on-call shift

Job Responsibilities
- Coordinate and oversee all direct care patient services provided by clinical personnel.
- Develop, plan, implement, analyze, and organize clinical operations for the location managed.
- Conduct and delegate the assessment and reassessment of patients, including updating care plans.
- Manage the assignment of caregivers.
- Oversee the delivery of care to all patients served by the location.
- Receive case referrals and review available patient information related to the case.
- Determine home health or hospice needs and ensure patients meet admission criteria.
- Assign appropriate clinicians to cases as needed.
- Instruct and guide clinicians to promote effective performance and quality home care services.
- Assist clinicians in establishing therapeutic goals and developing patient Plan of Care (POC).
- Monitor cases for compliance with regulatory agencies and requirements of third-party payers.
- Ensure audits and billing are completed timely and in compliance with Medicare regulations.
- Coordinate communication between team members, attending physicians, and caregivers.
- Establish location’s revenue and budget goals in conjunction with the Branch Director and Finance Department.
- Participate in sales and marketing initiatives.
- Supervise all clinical employees at the location.
- Handle employee corrective action and discipline in consultation with HR.
- Participate in the interviewing, hiring, training, and development of direct care clinicians.
- Evaluate clinician performance and recommend in-service education programs.
- Assess staff education needs and create regular staff education sessions as needed.
- Analyze situations and identify problems, evaluating alternative courses of action.
- Review case managers' documentation for adherence to care delivery and documentation requirements.
- Conduct QA/PI activities and work with Utilization Review staff for performance tracking.
- Perform direct patient care on an infrequent basis and in emergencies.
- Act as Branch Director in his/her absence.
- Ensure compliance with external regulatory authorities and company standards.
Required Skills
Core Skills
- Active and unrestricted RN license in Georgia
- Management experience in a home care or hospice environment
- OASIS experience
- Knowledge of CMS PDGM billing
- Strong communication skills
Preferred Skills
- Bachelor of Science in Nursing (BSN)
- OASIS certification (HCS-O, COQS, COS-C)
- Experience with Homecare Homebase (HCHB)
Requirements
Experience: Two years as a Registered Nurse with at least one year of management experience
Education: Associate Degree in Nursing (ADN) required; BSN strongly preferred
License & Certification:
- Active RN license in Georgia
- Current CPR card and/or BLS certification
Schedule:
- Full-time, 40 hours per week
- On-call expectation with a rotating schedule
