Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts initial assessment, re-assessment, and focused assessment based on patient condition, setting, and population.
Identifies problems based on clinical judgment about the patient’s response to health conditions or needs.
Sets measurable and achievable short- and long-range goals in collaboration with the patient and family.
Establishes daily goals with the patient and family.
Formulates a plan of care and ensures continuity of care during hospitalization and in preparation for discharge.
Documents care in the patient’s record and ensures patient safety.
Administers medications and provides ordered treatments.
Educates patients and families about care.
Continuously evaluates patient’s status and the effectiveness of nursing care.
Manages and corrects the sequence, timing, and effectiveness of patient care.
Delegates and supervises care tasks as needed.
Prioritizes patient care.
