Job Responsibilities
- Utilizes the nursing process to provide individualized nursing care.
- Collects and relays information to providers on both hospital and clinic patients enrolled in approved programs.
- Provides individualized patient education based on assessed needs.
- Develops and implements individualized care plans.
- Assists with the coordination of patient care across the continuum by collaborating with the inter-professional team and community partners.
- Manages disease registries by identifying and following up with patients not meeting clinical and evidence-based goals.
- Provides healthcare coordination of services and implements evidence-based practice to support overall goals of the designated setting and/or program.
- Applies population health care management strategies to panels of patients.
Required Skills
Core Skills
- Thorough knowledge of the nursing practice act and standards of nursing care.
- Knowledge of administrative and management techniques.
- Competency in using age and population specific standards in clinical setting.
Preferred Skills
- Professional nursing experience in a clinic care setting.
- In-depth knowledge of specialty area.
Requirements
Experience: Minimum of one year of professional nursing experience in a clinic care setting preferred.
Education: Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
License & Certification:
- Unencumbered Licensed Practical Nurse (LPN) license with State Nursing board
- Basic Life Support (BLS) certification required within six months of employment.
- Valid driver’s license.
