Job Title: RN Clinical Field Staff Liaison
Position: Clinical Liaison for Home Health
Job Type: Full Time
Pay: $38.00 - $42.00 based on experience. Sign-On Bonus $7,500
Shift: Monday-Friday 8:00 AM-4:30 PM

Job Responsibilities
- Serve as a resource for field staff, referral sources, physicians, patients and their families pertaining to clinical issues and/or concerns.
- Facilitate calls to and from field staff and use resources to resolve issues, reporting anything of concern to manager.
- Work in the field with staff and patients as needed.
- Maintain positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted livings and families.
- Assist with monthly Case Manager, Nurse, and Therapy meetings.
- Ensure Start of Care compliance.
- Ensure quality and safe delivery of home health services.
- Monitor Home Health Aide supervisory visits.
- Focus on Outcome Monitors as assigned.
- Implement quality improvement programs in the field.
- Complete field visits as needed.
- Participate in managing cost of goods by monitoring supply costs and visits per episode.
- Review relevant reports and clinical documentation in accordance with agency protocol and Medicare/Federal guidelines.
- Maintain active RN license in state of employment.
- Participate in on-call, evening/weekend shifts and provide patient care, per agency needs.
- Participate in in-service program development.
- Maintain a professional appearance as a representative of the company.
- Understand, implement and practice policies and procedures for agency operation.
- Other duties as assigned.
Required Skills
Core Skills
- Registered Nurse with current license in the state of employment.
- Three years of experience as an RN in an acute care setting.
- Ability to effectively communicate and create positive impressions with patients, families, physicians and co-workers.
- Ability to remain calm, patient, and accommodating.
- Knowledgeable on nursing best practices.
- Ability to make appropriate nursing judgments.
- Detail-oriented and observant.
- Disciplined work ethic with the ability to prioritize and be timely.
- Ability to follow directions and work as a team member.
- Knowledge of MS Office applications including Word, Excel, PowerPoint and Outlook.
Preferred Skills
- Minimum one year of home health experience is preferred.
- Valid driver’s license and auto insurance with your name as a listed driver.
Requirements
Experience: Three years of experience as an RN in an acute care setting, minimum one year of home health experience preferred.
Education:
License & Certification:
- Registered Nurse
- Compact licensure (for states that participate) to be obtained within first 30 days of employment.
Schedule:
- Monday-Friday 8:00 AM-4:30 PM
- On-call, evening/weekend shifts as needed.
