Job Responsibilities
- Perform comprehensive nursing history and physical on new referrals
- Create appropriate care plan and participate in IDT
- Evaluate and triage participants with episodic illnesses
- Refer participants to medical director or medical specialist as indicated
- Actively participate in family conferences
- Participate in interdisciplinary team meetings, committees, and work groups as assigned
Required Skills
Core Skills
- Bilingual English/Spanish
- Proven clinical leadership
- Ability to work in an interdisciplinary team environment
Preferred Skills
- Knowledge of Creole
- ACLS certification
Requirements
Experience: At least 1 year of RN or 3 years of LPN; Minimum one-year experience working with a frail/elderly population
Education: Graduate from an accredited school of nursing
License & Certification:
- Current RN license by the Florida State Board of Nursing
- Valid Florida Driver’s License & Proof of Insurance
- BLS certification
