Job Title: Care Manager - Home Care (RN)
Position: Care Manager for High-Risk Patients
Job Type: Full Time
Pay: $95,000 - $100,000 / year
Shift: M-F 8:30 AM - 5 PM, on-call participation

Job Responsibilities
- Develop and implement individualized care plans for high-risk patients.
- Coordinate patient care with interdisciplinary teams, including physicians and social workers.
- Monitor patient progress, track health metrics, and adjust care plans as needed.
- Provide advocacy and support to patients and families.
- Educate patients and families on disease management and medication adherence.
- Conduct comprehensive in-home assessments to evaluate patients' needs.
- Perform hands-on care interventions, including medication management.
- Assess the home environment for safety and implement necessary changes.
- Provide education and training to patients and caregivers.
- Work with Home Care NP or PA to ensure adherence to care plans.
- Empower patients and their caregivers by providing tools for managing conditions.
- Offer counseling and support for chronic illness or end-of-life care.
- Collaborate with multidisciplinary care teams to coordinate patient care.
- Participate in regular team meetings to discuss patient progress.
- Contribute to continuous improvement initiatives within the home care program.

- Massachusetts RN license due to regional travel
