Job Responsibilities

  • Coordinates Clinical Practice and Clinical Program Services; manages a direct-care caseload to provide care to patient populations identified by the primary care physician.
  • Enter pre-visit orders and prepare forms (such as FMLA, FL2, PCS, CAP-C, CC4C, MAW and allergy/asthma action plans, etc.) as needed.
  • Assist with the development and maintenance of clinic Chronic Care Disease Registry.
  • Designs and Coordinates Education Programs for families and patients to assist with care outside of clinic.
  • Participates in various clinic committees to organize and improve patient care and serves as liaison between patients and providers.
  • Monitor Maestro in-basket for patient call notifications.
  • Respond immediately to semi-urgent and urgent messages received by pager and return patient calls.
  • Collaborate with triage & providers and communicate the plan of care to patients.
  • Document patient communication in telephone encounters.
  • Using the Nursing Process, collect and analyze patient data, make recommendations for nursing care plans and evaluate nursing care provided.
  • Document and communicate appropriately nursing actions taken and patient responses.
  • Serves as a resource to patients and families, providing counseling and follow-up problem solving.
  • Assist providers with patient appointments and provide onsite coordination in the clinic.
  • Collaborate with social work, Duke Well, and other members of the patient care team for patient needs.
  • Coordinates the development, utilization, and evaluation of appropriate educational resources and interventions that assist the patient/caregiver in developing the skills for self-care.
  • Participates in quality/performance improvement programs and projects, product evaluation, outcomes evaluation studies and/or clinical research.
  • Perform other related duties incidental to the work described herein.

Requirements

Experience: At least three years demonstrated clinical expertise
