Provide resident-centered nursing care and daily living assistance to assigned residents under the supervision of a registered nurse (RN).
Deliver age-appropriate care related to the physical and psychological needs of the resident as per care plan.
Assist with basic health care needs including bathing, toileting, grooming, dressing, obtaining and recording vital signs.
Provide psychosocial support and other personal care to assigned residents.
Assist residents in transferring, repositioning, and walking using correct and appropriate techniques.
Complete tasks related to food preparation and dining, ensuring a positive dining experience.
Assist residents with meals and snacks, providing water and supplements as appropriate.
Aid providers and nursing staff members with procedures as needed.
Document resident interactions and outcomes related to care, behavior, activity, and dietary.
Communicate resident's changing conditions and care concerns to the charge nurse.
Collect non-invasive body fluid specimens and gather vital signs.
Provide housekeeping and laundry services to assist residents.
Support wellness and recreational programs, encouraging resident participation.
Participate in departmental/organization committees and task forces as needed.
Complete tasks related to the Restorative Nursing program, including documentation and leading restorative programs.

Experience: Two years' clinical or nursing assistant experience in long-term care preferred.

Education: High school graduate or general equivalency diploma (GED) preferred.

License & Certification:

Certified Nursing Assistant (CNA) certification with the State Board of Nursing
Certified and registered with the Unlicensed Assistive Person (UAP) Registry (if applicable)
