- Perform start-of-care visits and comprehensive assessments (including OASIS)
- Develop and initiate individualized care plans
- Educate patients and families on the care process and expectations
- Collaborate with a supportive interdisciplinary team

- At least one year of clinical experience
- Strong assessment and documentation skills
