Conduct home visits in assigned territory to assess home safety, gather changes in health status, and provide clinical coaching and support including review of medications.
Provide ongoing coaching and support to caregivers via an app, including resource referral and reinforcement of best practices according to an established care plan.
Develop trusting relationships with caregivers to influence change and reduce caregiver burnout.
Document and review records for accuracy and timeliness, ensuring compliance with regulatory and accreditation standards.
Perform other duties as assigned.

Active Registered Nurse (RN) License in the state of employment
Minimum two years of recent experience in direct care of elders or adults with disabilities
Experience in case management, care planning, and assessment
Proficiency in common software and web-based applications (e.g., MS Office, case management systems)
