Job Responsibilities
- Performs initial, ongoing, and functional health status assessments as applicable to the population and/or individual.
- Documents planned nursing interventions to achieve outcomes appropriate to patient needs based on nursing diagnoses and collaborative problems.
- Evaluates and documents response to nursing interventions and achievement of outcomes at appropriately determined intervals.
- Revises plan of care based on evaluative data as part of a multidisciplinary team.
- Actively participates in process improvement activities to achieve targeted measures of clinical quality, customer satisfaction, and financial performance.
- Performs other duties as requested by supervisor.
