Job Responsibilities

Coordinate care and services of selected patient populations through the acute care episode and across the continuum.
Work collaboratively with interdisciplinary teams and team members both internal and external to the organization.
Improve patient care by assuming a leadership role in driving desired clinical and financial outcomes.
Utilize healthcare resources effectively through monitoring and utilization.
Incorporate data to continuously improve care and outcomes.

General knowledge of payor industry (Medicare, Medicaid, & Commercial insurance regulations)

Preferred Skills

Accredited case manager certification (ACM-RN) through the American Case Management Association (ACMA)
Experience in transitional planning, outcomes management, and assessment
5 years of clinical nursing and case management experience

Requirements

Experience: A minimum of 3 years of clinical nursing; 5 years preferred

Education: Minimum of two years education at an accredited School of Nursing; Bachelor's degree preferred

License & Certification:

Current state of Wisconsin Registered Nurse License
Multi-state Nursing License from a participating state in the NLC
