Job Responsibilities

  • Provides case management, advocacy, and care coordination services to hospital patients in various settings to ensure seamless transitions in care while minimizing delays or risk of readmission.
  • Conducts early patient assessment to identify coordination and discharge planning needs.
  • Creates individualized, patient-focused plans for meeting continuing care needs.
  • Makes referrals to post-hospital care providers using knowledge of community resources.
  • Collaborates with the healthcare team to ensure timely discharge, complete patient education, and established follow-up.
  • Utilizes current clinical nursing knowledge to educate patients and families about chronic disease conditions.
  • Collaborates with staff nurses and physicians to ensure all core measure elements are met prior to discharge.
  • Conducts clinical review processes according to the Utilization Management Plan.
  • Identifies and intervenes in situations that pose financial risk to the patient and the organization.

Required Skills

Core Skills

  • Registered Nurse License
  • CPR certification
  • Knowledge of Allscripts Care Management program
  • Expertise in Epic EMR
  • Microsoft Outlook proficiency
  • Basic knowledge of Excel and Word

Preferred Skills

  • Case Management Certification (ACM)
  • Specialty certification in Case Management, Med/Surg, or Diabetic Education within three years

Requirements

Experience: Specific years of applicable experience may be required per department guidelines.

Education: Graduate of an accredited nursing program; BSN or BSN completion agreement upon hire.

License & Certification:

  • Current Registered Nurse License in the state of practice
  • CPR certification

Schedule:

  • Availability to work on-call on weekends and holidays as assigned.
