Job Responsibilities
- Serve as the coordinator for the care of patients in their specific specialties.
- Provide support, education, referrals, coordination of care, and other assistance to the patient and their family prior, during, and following their healthcare experience.
- Facilitate integrated care through communication with strategic members of the interdisciplinary team.
- Collaborate to provide staff education and work closely with patients, families, and members of the healthcare team to optimize patient satisfaction and achieve optimal outcomes.
- Coordinate efforts between physicians, healthcare administration, family, community resources, and staff while tracking the progress of the patient throughout their continuum of care.
- Promote evidence-based clinical care that exemplifies the pursuit of clinical excellence.

Requirements
Experience: 2 years in Acute Care or related field
