Job Responsibilities
- Deliver direct dementia care and build strong relationships with patients and families.
- Conduct cognitive, behavioral, and medication assessments.
- Participate in team meetings, case conferences, and care coordination.
- Support caregivers and assist in advanced care planning.
- Stay current with evidence-based dementia treatments.
Required Skills
Core Skills
- Strong assessment, diagnostic, and communication skills.
- Experience in geriatrics, palliative, or chronic care.
Preferred Skills
- 1–2+ years with older adults preferred.
- Telehealth and EHR experience (Athena a plus).
Requirements
Experience: 1–2+ years with older adults preferred.
Education:
License & Certification:
- Active, unrestricted NP license (FL, TX, or WA)
- Board certified
- DEA license (or eligibility)
Schedule:
- 4x10-hour days, minimal weekends/after-hours
