Job Title: Licensed Practical Nurse (LPN) - Hospice Per Diem - $33 to 38/hr
Position: Per Diem opportunity supporting in-home patients
Job Type: Contract
Pay: $33.00/hr for days, $38.00/hr for nights
Shift: Days - 8 to 12 hour shifts; Nights - 8 to 12 hour shifts

Job Responsibilities
- Administers nursing care according to the plan of care to terminally ill patients of all ages in their place of residence.
- Coordinates care with the interdisciplinary team, patient, family, and referring agency in accordance with the Nurse Practice Act and state law.
- Evaluates patient/family needs and assists in the preparation and execution of the plan of care for each patient under the direction of the Registered Nurse.
- Revises and resolves patient problems and goals as changes occur and/or recertification occurs after consultation with the Registered Nurse.
- Participates in providing bereavement support to families, including closure contacts and associated bereavement activities as assigned by the team.
Required Skills

- Active FL State or compact LPN License
- Experience in oncology, long-term care, hospice, or home health
