Job Responsibilities
- Provide skilled nursing services under the supervision of a Registered Nurse as ordered on the plan of care.
- Instruct patients and caregivers in various aspects of patient care and disease management.
- Document patient and caregiver comprehension and compliance with teaching.
- Observe patient response to care and teaching, making recommendations to the Registered Nurse for revisions in the care plan.
- Provide and document skilled nursing care according to the established plan of care, policies, procedures, and standards of care.
- Perform other duties as assigned by the RN / Clinical Manager.
Required Skills
Core Skills
- Unencumbered, current Licensed Practical Nurse license
- Graduation from a state-approved LPN program
- Minimum one year of clinical experience as an LPN
- Experience in home health care is desirable
