Job Responsibilities
- Serves as a single point of contact for patients with cancer, their family members, caregivers, and referring physicians.
- Manages care of identified cancer patients in collaboration with healthcare professionals to ensure timely, high-quality care.
- Ensures seamless access to oncology care by assessing patient financial, emotional, social, and clinical needs throughout the illness trajectory.
- Connects patients and families to needed resources.
- Organizes and facilitates Multidisciplinary Clinics.
- Participates in Cancer Institute and community disease-specific outreach events.
- Accompanies patients to provider appointments to provide support, education, and reinforcement of information.
- Supports the clinical trial team through documentation and submissions.
- Works with patients to ensure access to oncology care by assessing medical and psychosocial needs.
- Participates in community outreach activities promoting cancer support services and educational workshops.
- Facilitates prevention activities such as nutrition planning, exercise, and smoking cessation.
- Provides information and refers oncology patients to resources within the health network and the community.
- Identifies patients for referral to a multidisciplinary team using standardized screening tools.
- Gives supportive resources to patients and families to promote a positive outlook.
- Collaborates with the Health Care Team to develop patient's plan of care.
- Facilitates transitions in care by collaborating with other programs and departments.
- Monitors patient utilization of express care, ER, and inpatient services and develops plans for utilization reduction.
- Monitors patient care compliance and outcomes.
- Acts as patient liaison to facilitate communication between patient, family, and Health Care Providers.
Required Skills
Core Skills
- Bachelor’s Degree in Nursing
- 2 years RN oncology experience
- Ability to advocate for patients
- Ability to manage a clinical caseload of cancer patients
- Current knowledge of clinical practices and related information systems
Preferred Skills
- 5 years RN oncology experience
- Bilingual skills (Spanish and English)
Requirements
Experience: 2 years RN oncology experience
Education: Bachelor’s Degree in Nursing
License & Certification:
- OCN - Oncology Certified Nurse
- RN - Licensed Registered Nurse in Pennsylvania
- American Heart Association Basic Life Support
