Position: Health Coach supporting care transitions and patient engagement
Job Type: Full Time
Pay: $2500.00 sign on bonus
Shift: Monday through Friday 8am - 4:30pm

Job Responsibilities
- Navigate high-risk patients across the continuum.
- Serve as the main point of contact for patients and providers across care settings.
- Aim to better manage patients in ambulatory settings and reduce acute care and ED use.
- Develop patient care plans with interventions and goals appropriate to patient needs.
- Collaborate with both internal and external entities to facilitate seamless transitions of care.
- Initiate and complete the transition of care (TOC) process at time of patient discharge.
- Assist patients with psychosocial needs and help access benefits and community resources.
- Connect patients using ED for non-acute needs with primary care and promote appropriate site utilization.
- Provide self-management support and disease-specific guidance.
- Manage patients' discharges from in-patient facilities and ED, including medication reconciliation and follow up.
- Utilize motivational interviewing skills to promote patient engagement and activation.
- Establish rapport with patients and assess their readiness for change.
- Communicate lab and other test results to patients in collaboration with provider and other office staff.
- Link patients to community resources and assist with quality improvement activities.
- Initiate improvement projects in clinics.
- Monitor quality measures and coordinate the completion of payer gap lists.
Required Skills

- Knowledge of medical terminology and procedures

- Ability to provide caring patient care services
- Ability to critically listen and effectively communicate
Preferred Skills
- Experience with motivational interviewing
- Knowledge of recognized diseases and healthcare practices
Requirements
Experience: Minimum 1-3 years of patient care experience required.
Education: Graduated from an accredited school of nursing required, Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred.
License & Certification:
- Current valid Registered Nurse (RN) license
- American Heart Association or American Red Cross Basic Life Support (BLS) preferred at time of hire, required within 3 months of hire
