Job Responsibilities

Assess and identify patient/caregiver problems.
Establish patient/caregiver care goals.
Identify and implement interventions.
Work with attending physician to establish and implement medical plan of care for the patient.
Establish and maintain therapeutic relationship with patient and caregiver, including providing emotional support and teaching.
Work closely with other members of the interdisciplinary team.
Communicate effectively both verbally and in writing with team members, patients, caregivers, physicians, pharmacists, and other health care professionals and community service organizations.
Supervise Hospice Aide on shift.

Required Skills

Core Skills

CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS)
Nursing skills relevant to hospice and palliative care
Effective communication skills

Requirements

Experience: One year of experience in an acute, hospice, home health setting, or long-term care setting.

Education: Graduate of an accredited School of Nursing.

License & Certification:

Currently licensed as an RN in North Carolina
Valid state-issued driver's license
