- Coordinate the Minimum Data Set (MDS) process for the facility
- Ensure resident-centered care model is followed
- Support clinical staff and manage care plans
- Monitor census and ensure staffing ratios are maintained

- Extensive knowledge of RAI process
- Ability to multi-task
Preferred Skills
- RAC-CT Certification
- 2+ years of MDS experience in skilled nursing/long-term care
Requirements
Experience: 2+ years of MDS experience in skilled nursing/long-term care
Education: Registered Nurse (RN) in New Hampshire (active and in good standing)
License & Certification:
- RN in New Hampshire
- RAC-CT Certification preferred
