Position: Registered Nurse in TeleCare and Remote Patient Monitoring

Job Responsibilities

  • Triage and prioritize patient care using Evidence Based Practice Standards.
  • Effectively identify and resolve urgent patient clinical inquiries.
  • Monitor Remote Patient values out of range requiring intervention.
  • Provide quality patient care in compliance with regulations and accreditation standards.
  • Accurately assess each patient's condition by actively listening and asking questions.
  • Identify emergent situations and determine appropriate dispositions.
  • Provide patient education and plan of care to callers according to approved protocols.
  • Document all telephone conversations accurately and immediately after the call.
  • Place follow-up calls to caregivers as necessary.
  • Communicate essential information to the physician on call.
  • Meet unit expectations for call volumes, duration, and customer service.
  • Work autonomously while adhering to organizational standards.
  • Attend and participate in staff meetings and required departmental meetings.
  • Utilize and maintain supplied equipment while troubleshooting tech issues.
  • Develop individualized plans of care based on patient assessments.
  • Demonstrate knowledge of growth and development principles for patient care.
  • Provide, delegate, coordinate, and document patient care activities.
  • Communicate patient condition or unit concerns to healthcare team members.
  • Monitor and respond to triage calls in person and/or on the nurse phone line.
  • Direct patients to appropriate care settings based on assessments.
  • Ensure accurate documentation in the patient's Electronic Health Record.
  • Assist in making appointments for patients who do not require ER visits.
  • Consult with physician as needed.
  • Act in the 'Ask a Nurse' capacity for routine information requests.
  • Serve as a clinical resource and role model for nursing staff.
  • Prioritize work tasks and complete them timely.
  • Assist with medical chart reviews and data compilation for audits.
  • Assist in developing nursing policies and procedures.
  • Provide patient education regarding disease processes and therapies.
  • Maintain a safe and caring environment for patients and families.
  • Integrate patient and family education into care delivery.
  • Participate in Process Improvement activities as assigned.
  • Maintain compliance with legal, regulatory, and accreditation standards.
  • Ensure regular, punctual attendance conforming to policies.

Requirements

Experience: One (1) year of hospital RN experience required, preferably in the ED Unit.

Education: Bachelor of Science in Nursing (BSN) degree.

License & Certification:

  • Current RN license in the State of Florida or Compact Licensure
  • Current BLS/Healthcare Provider certification
  • ACLS as required based on patient population
