Job Responsibilities

Completes comprehensive assessments of members per regulated timelines.
Determines qualifications for case management based on clinical judgment, health changes, and assessment triggers.
Develops and implements a case management plan with members, caregivers, physicians, and healthcare professionals.
Conducts face-to-face or home visits as required.
Performs ongoing monitoring of care plans for effectiveness and suggests changes as necessary.
Maintains ongoing member case load for outreach and management.
Promotes integration of services for behavioral health care and long-term services.
Facilitates interdisciplinary care team meetings and informal collaboration.
Uses motivational interviewing to educate and support members.
Assesses barriers to care and provides coordination and assistance to members.
Provides consultation and education to non-RN case managers.
Handles cases with complex medical conditions and conducts medication reconciliation when needed.

Requirements

Experience: 1-3 years in case management, disease management, managed care or medical or behavioral health settings

Education: Graduate from an Accredited School of Nursing; Bachelor's Degree in Nursing preferred

License & Certification:

Active, unrestricted State Registered Nursing (RN) license in good standing
Valid driver's license with good driving record

Schedule:

25-40% local travel required
