Provides personal care and related activities within scope of practice for designated patients in strict accordance with the written plan of care.
Assumes responsibility for and completes routine and complex assignments for assigned home care/hospice patients within the allotted period of time.
Completes documentation of patient care provided and communicates with other disciplines as appropriate.
Observes changes in patient functional status and personal care needs; documents observations and reports changes and/or findings to supervising clinician or clinical manager.

Strong relationship-building skills with patients, family members, caregivers, and interdisciplinary care team.
Knowledge of medical terminology.
Strong time management skills.
Ability to drive to patients' homes regardless of weather and traffic conditions.
Ability to work with and support terminally ill patients and their family members/caregivers.

Experience: 1 year experience in a healthcare setting as a nursing assistant or nursing aide.

Education: High School Diploma/GED
