Shift: Friday - Friday; 7 on 7 off, 5:00 PM Friday - 8:00 AM Monday, 5:00 PM - 8:00 AM Tuesday - Friday

Perform comprehensive nursing evaluations and after hour assessments on patients with a change in condition.
Assess for signs and symptoms of discomfort including presence of pain and take prompt and appropriate action.
Initiate plan of care as needed and appropriately supervise patient care after hours, providing appropriate nursing follow-up to all patients.
Instruct Caregivers in managing patient needs and submit appropriate documentation within 24 hours of visit.
Coordinate all services provided to patients/families in all settings to ensure quality of care and services as well as appropriateness of care.
Follow the approved plan of care, modify as needed, and contact physician when appropriate.
