Job Responsibilities

Provide patient care to infants, children and young adults requiring specialized care for chronic conditions related to developmental disabilities and mental health.
Coordinate care of patients/families in the clinic.
Provide triage services and monitor overall clinic flow and processes.
Assess patients' nursing care needs and evaluate effectiveness of care provided.
Integrate nursing theory and research into clinical practice.
Provide leadership to nursing team members to maintain patient care standards.
Develop patient care plans based on condition, age, and other factors.
Implement patient care plans and document/report interventions and patient/family responses.
Collaborate and integrate contributions of patients and families into multidisciplinary care plans.
Implement nursing interventions and follow department protocols to maintain a safe environment.
Act as a liaison for patients/families, clinicians, and community resources.

Baccalaureate Degree in Nursing or Master's Degree in Nursing
Experience with chronic conditions, developmental disabilities, and mental health

Knowledge of community programs for people with disabilities
Experience with electronic medical record programs

Requirements

Experience: Previous nursing experience working with pediatric patients; one year experience in a patient care setting that included triage responsibilities.

Education: Associate’s Degree, Diploma in Nursing, Baccalaureate Degree, Professional Masters of Nursing and Healthcare Practice (MNHP), or Master’s Degree in Nursing (MSN, MA) is required.

License & Certification:

Current license to practice nursing in Iowa
