Job Responsibilities

Provide direct care for all assigned patients, including initiation and maintenance of communication with appropriate care team members.
Evaluate and educate patient/care partner on safely administering home infusion therapy and provide additional instruction as needed.
Evaluate the home environment for safety and appropriateness for prescribed care and identify necessary interventions.
Develop and evaluate the plan of care in conjunction with team members during each encounter.
Maintain caseload scheduling via the company-approved platform per company policy.
Communicate scheduling changes to appropriate team members as needed.
Coordinate care via the approved company platform.
Participate in the nurse on-call rotation to ensure continuous nursing support services.
Document all communications at the time of service per company policy.
Ensure timely submission of treatment plans as required by state guidelines.

Required Skills

Core Skills

Strong knowledge of therapies provided by Soleo
Proficient with Microsoft Outlook and Microsoft Office Suite

Preferred Skills

Advanced certification in specialty areas of practice (e.g., CRNI, ONS, IgRN) preferred
Experience in ICU and/or Cardiac Care preferred

Requirements

Experience: Minimum 3 years nursing experience with background in infusion therapy preferred.

Education: RN Degree, BSN Preferred

License & Certification:

Current nursing license for applicable home state

Schedule:

On-call requirement may include nights and weekends.
