Position: Primary Care Provider for Senior Population
Job Type: Full Time

Job Responsibilities
- Own the patient's quality of care and collaborate with attending physicians and other providers.
- Coordinate patient care with the facility Director of Nursing and attending physicians.
- Document patient care in the appropriate EMRs.
- Function as the primary provider and treat an average of 12 to 16 patients per day.
- Take initiative and collaborate with other MPAC providers to ensure optimal patient care.
- Build trust and rapport with facility personnel and attending physicians.
- Make high-risk decisions confidently in ambiguous situations.
- Identify and handle strategic issues.
- Foster open communication and teamwork with the facility team and Regional Clinical Manager.
Required Skills
Core Skills
- Nurse Practitioner License
- ANCC or AANP Certification
- Clinical knowledge in post-acute medicine
Preferred Skills
- Experience in a nursing home setting
