Job Responsibilities
- Develops, assesses, and facilitates complex care management activities for primarily physical needs members.
- Provides high quality, cost-effective healthcare outcomes, including personalized care plans and education for members and their families.
- Evaluates the needs of the member and barriers to accessing appropriate care.
- Focuses on social determinants of health needs as identified by the member.
- Recommends and/or facilitates the best outcome care plan.
- Develops ongoing care plans/service plans and collaborates with providers to identify specialists/community resources.
- Identifies problems/barriers to care and provides appropriate care management interventions.
- Coordinates between the member/family/caregivers and the care provider team for person-centered care.
- Provides ongoing follow-up and monitoring of member status and progress towards goals.
- Collaborates with member, caregivers, and providers to revise care plan as necessary.
- Provides resource support for members regarding various services based on assessments.
- Facilitates care management and collaborates with providers to ensure timely access to care.
- May perform telephonic, digital, home, or site outreach to assess member needs.
- Documents and maintains all member information and care management activities for compliance.
- Provides education to members and families on disease processes and healthcare options.
- Gives feedback to leadership on improving care and quality delivery.
Required Skills
Core Skills
- Registered Nurse (RN) licensed in Florida
- Pediatric experience
- Bilingual (Spanish-English) preferred
Preferred Skills
- Experience with care management
- Knowledge of healthcare regulations
Requirements
Experience: 2 - 4 years of related experience in pediatric care
Education: Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
License & Certification:
- Registered Nurse (RN) licensed in the State of Florida
Schedule:
- May require up to 80% local travel
