Job Responsibilities
- Works directly with the clinical and sales team to identify member external appointments and facilitates access to those providers.
- Uses facility EMRs to prescreen new enrollees for continuity of care needs for external appointments and facilitates their access.
- Performs care coordination activities related to member transitions of care communication to and from facility.
- Assists with Prior authorization requests process.
- Aids members needing out-of-nursing facility care and services.
- Knows how to access the drug formulary and provider directory.
- Communicates with member patients, families, and doctors as delegated by Nurse Practitioners.
- Participates in Longevity Health Plan NC standing clinical meetings periodically.
- Assists with gathering data for hospitalization review sessions.
- Contributes to onboarding new nursing homes and educating team members on LHP model.
- Assists in initiating telehealth services in newly onboarded nursing homes.
- Conducts nursing home orientation sessions on LHP MOC.
- Assists with LHP EMR data input as requested.
- Performs other duties as assigned based on caseload and workload.
Required Skills
Core Skills
- Excellent verbal and written communication skills.
- Excellent computer skills, including EMR/clinical platforms and MS Office products.
- Managed care coordination experience.
Preferred Skills
- Nursing home background or health plan case management experience.
- Minimum 2 years experience as a Registered Nurse.
- Knowledge of challenges and opportunities in serving long-term nursing home populations.
Requirements
Experience: Minimum 2 years experience as a Registered Nurse.
Education: Associate degree in nursing required; bachelor’s degree preferred.
License & Certification:
- Licensed registered professional nurse in good standing.
- Valid state driver's license with a good driving record.
- Proof of automobile insurance required.
