Position: Full-time (40hr per week) Nurse for STRIDE Street Medicine Program
Job Type: Full Time
Pay: $72,000 - $96,000
Shift: Monday-Friday, 8 hours/day

Job Responsibilities
- Provides nursing care outreach in encampments and streets to highly vulnerable people experiencing homelessness.
- Provides nursing care at respite, shelters, micro-communities, health fairs, school based events/outreach.
- Implements care plans and provides urgent and primary care services within a licensed Registered Nurse scope of practice.
- Collaborates in managing common health care problems present in the community, such as diseases of neglect and exposure, substance abuse related illnesses, cognitive impairments, chronic illnesses, infectious diseases, musculoskeletal problems, and psychiatric disease.
- Performs timely client/patient triage assessment with clinical competence and judgment for urgent/drop-in-care.
- Provides education and counseling on various health topics including pregnancy counseling, contraception, and mental health conditions.
- Performs initial and continued assessment of client health status, immunization, and TB screening.
- Provides outreach, information, referral, care coordination, and case management services.
- Implements innovative interventions to improve care of high-risk individuals experiencing homelessness.
- Uses multiple EMR platforms to gather patient information and maintains medical records.
- Communicates effectively with clients, clinic staff, and the public in a non-discriminatory manner.
- Participates in Street Medicine clinical case conferences and other meetings.

Experience: Minimum of one year experience in community health as a Registered Nurse

- Current and valid Colorado Registered Nurse license
