Position: Oversees all direct care patient services and clinical operations.

Pay: $93,900 - $129,300 per year

Job Responsibilities

  • Coordinate and oversee all direct care patient services provided by clinical personnel.
  • Develop, plan, implement, analyze, and organize clinical operations for a specific location.
  • Conduct and delegate the assessment and reassessment of patients, updating care plans and interpreting patient needs.
  • Manage the assignment of caregivers and oversee the delivery of care to all patients.
  • Receive and review case referrals to determine home health or hospice needs.
  • Ensure patients meet admission criteria and make admission decisions.
  • Assign appropriate clinicians to cases as needed.
  • Instruct and guide clinicians to promote effective performance and delivery of quality home care services.
  • Assist clinicians in establishing immediate and long-term therapeutic goals and patient Plan of Care.
  • Monitor cases to ensure documentation is in compliance with regulatory agencies and requirements of third-party payers.
  • Ensure audits and billing are completed timely and comply with Medicare regulations.
  • Coordinate communication between team members, attending physicians, and caregivers.
  • Work with the Branch Director and Finance Department to establish revenue and budget goals.
  • Participate in sales and marketing initiatives.
  • Supervise all clinical employees assigned to a specific location.
  • Handle employee corrective action and discipline issues fairly and objectively.
  • Participate in interviewing, hiring, training, and development of direct care clinicians.
  • Evaluate staff performance relative to job goals and requirements.
  • Assess staff education needs and create regular staff education as needed.
  • Analyze situations, identify problems, and evaluate courses of action using Performance Improvement principles.
  • Review documentation for starts-of-care, resumption-of-cares, and re-certifications.
  • Conduct QA/PI activities and track performance review data.
  • Perform patient care assignments during emergency situations.
  • Act as Branch Director in absence.
  • Ensure compliance with external regulatory authorities and internal documentation standards.
  • Participate in performance improvement activities and maintain ongoing clinical knowledge.
  • Maintain relationships with referral and community sources.
  • Conduct care-related programs and participate in professional organizations.

Requirements

Experience: Two years as a Registered Nurse with at least one year of management experience.

Education: Graduate of an accredited School of Nursing.

License & Certification:

  • Current state license as a Registered Nurse
  • Current CPR certification
  • Valid driver’s license and auto insurance
