Provide coordination of care for patients and families.
Ensure continuous assessment of each patient's and family's needs.
Implement the interdisciplinary care plan.
Perform services in accordance with physician’s orders and the established plan of care (POC).
Ensure delivery of care is compliant with federal and state regulations.

Experience: At least one year experience as a licensed registered nurse (RN)

Education: Must be a graduate of an approved school of nursing education
