Position: LPN for the PACT Clinics

Job Responsibilities
- Performs assigned nursing care responsibilities under limited to moderate supervision.
- Documents vital signs including pulse, blood pressure, respirations, temperature, and pain level.
- Completes clinical reminders for preventive health screening including mental health screening.
- Performs diabetic foot screening including visual screening, pulses, and sensation.
- Administers vaccinations including influenza, pneumococcal, tetanus, COVID-19, and others per protocol.
- Refers patients to appropriate resources including social work, dietitian, speech pathology, and therapy.
- Performs clinical measurements such as Glucometer measurements and neurological vital signs.
- Adheres to standards of practice and safe technique in medication preparation and administration.
- Procures sterile and non-sterile specimen collections.
- Prepares equipment and administers Oxygen Therapy.
- Applies dressings and performs irrigations.
- Provides patient/family education for conditions and preventive health care.
- Communicates with difficult patients under limited to moderate supervision.
- Participates in training for clinic equipment and materials.
- Provides supportive care to patients and helps in admissions and transfers.
- Collaborates with the interdisciplinary team on the patient's plan of care.
- Utilizes the computer to access patient data and document progress notes.

Requirements
Experience: None beyond the basic requirements; GS-3 level applicants should have no additional experience.
Education: Graduate of a school of practical or vocational nursing approved by appropriate State agency.
License & Certification:
- Full, active, current and unrestricted licensure as a licensed practical or vocational nurse.
