Provide supportive services to participants in home or community settings, during transport, on appointments, or in day health center.
Assist in providing and maintaining optimal physical and emotional comfort to frail elders.
Support participants and their families in achieving self-reliance and independence.
Perform care or services outlined in individual participant’s Care Plan.
Assist with personal care and activities of daily living.
Help with meal preparation, feeding, toileting, and managing incontinence.
Provide simple treatments, shopping, medication reminders, laundry services, and homemaking services.
Offer extended hours care and transportation for off-site appointments.
Document care and services provided accurately and timely.
Report customer service complaints and scheduling deviations to supervisor.
Respond to emergency situations in accordance with established policies and procedures.

Certification in relevant fields (Direct Care, Nurse Assistant, etc.).
Ability to relate to participants with dementia and/or developmental disabilities.

Experience: 1 year with frail or elderly population in long-term care, hospital, or home care setting.

Education: High School diploma or GED required.

COVID-19 Requirements: Must be medically cleared for communicable diseases and have all immunizations up-to-date.

PCA competency monitored through skills evaluations and mandatory in-service training.
Ability to work sensitively with individuals of diverse ethnic and cultural backgrounds.
