Job Title: Primary Care Provider - Integrated Clinic
Position: Medical Doctor, Nurse Practitioner, Physician Assistant, or Naturopathic Doctor
Job Type: Full Time
Pay: Nurse Practitioner: $135,000 annually, Physician Assistant: $135,000 annually, Naturopath: $142,000 annually, Physician (MD/DO): $231,000 annually

Job Responsibilities
- Provide primary, urgent, and addiction medicine to underserved adults in a trauma-informed, recovery-focused setting.
- Play a vital role in Medication Supported Recovery through the low-barrier M3 buprenorphine/naloxone program.
- Support the Hepatitis C Treatment Program, helping patients get well and stay well.
- Work as a member of the Blackburn Center multi-disciplinary team to deliver comprehensive, client-centered and trauma informed integrated services.
- Identify new opportunities to improve collaboration between service and care teams while following information-sharing guidelines.
- Build understanding of and appreciate diversity among Blackburn Center clients and support their connection to culturally responsive services.
- Determine and formulate health histories and perform physical exams.
- Order, interpret and evaluate diagnostic tests to identify and assess patients’ clinical problems.
- Develop and implement person-centered care plans.
- Conduct ongoing healthcare and clinical management of stable, chronically ill patients.
- Provide services in an Office-Based Opioid Treatment program.
- Synchronize with the integrated care team to provide cost-effective, high-quality care.
- Assess and perform clinical procedures as necessary.
- Participate in and lead Provider Education Sessions on various clinical topics.
- Adhere to all state and federal privacy regulations, including HIPAA and patient confidentiality agreements.
Required Skills
Core Skills
- Demonstrated skills in primary care medicine
- Knowledge of trauma-informed care and diversity principles
- Strong communication and interpersonal skills
- Ability to work in a multi-disciplinary, team-oriented environment
Preferred Skills
- Experience with Hepatitis C primary care-based treatment
- Wound care experience
- Fluency in Spanish
- Training in Buprenorphine prescribing
- Knowledge of de-escalation methods
Requirements
Experience: Minimum of 2 years’ experience preferred in primary care medicine
Education: Graduate of an accredited Nurse Practitioner Program, Physician Assistant program, or Naturopathic Medical program; must have completed a CNME-certified primary care residency.
License & Certification:
- Current Advanced Practice Registered Nurse (APRN) Licensure or Licensed to practice as a Physician’s Assistant in Oregon.
- DEA Certification required or ability to obtain within 30 days of employment.
- BLS certification

- Flexibility to meet clients' needs during standard clinic hours
