Job Responsibilities
- Conduct comprehensive assessments and develop individualized care plans.
- Provide nursing care, medication administration, and health education.
- Monitor and reassess patient needs, adjusting care plans accordingly.
- Collaborate with physicians, families, and support staff.
- Supervise PCA/home health aides and mentor new RN Case Managers.
- Maintain accurate documentation and fulfill certification/recertification requirements.
- Participate in on-call duties and quality improvement initiatives.

- Experience with pediatrics and geriatrics
- Ability to treat wounds, foleys, sutures, and amputations
Requirements
Experience: Experience in home health care preferred

- Active Registered Nurse (RN) License
- Two-Step TB Test
- DHS Criminal Background Clearance
- Valid Driver’s License & Auto Insurance
Schedule:
- Monday through Friday
- On-call weekends
