Performs comprehensive nursing assessment/reassessment.
Conducts age-appropriate admission or transfer assessments.
Obtains input from family/guardian when appropriate.
Documents findings accurately and completely.
Assesses post-op/post-invasive procedure patients.
Documents education and discharge needs of patients and families.
Establishes, coordinates, and evaluates a plan of care.
Formulates nursing interventions based on patient care needs.
Collaborates with health team members for coordination of care.
Administers and documents medications accurately.
Monitors and maintains accurate IV fluids and parenteral nutrition.
Documents 'Readback' for all telephone/verbal orders.
Recognizes changes in patient's condition and takes appropriate nursing actions.
Involves family/guardian in care and decision-making when appropriate.
Maintains adherence to infection control practices.
Evaluates the effectiveness of care and modifies the plan as needed.
Documents ongoing evaluations of patient care and education.
Provides specialized care to patients at high risk for injury.
Initiates/evaluates alternatives to restraints prior to application.
Assesses patients for pain and implements pain management techniques.
Demonstrates accountability for professional practice.
Participates in continuing education for professional competence.
